- Address: Include city, state, and ZIP.
- Room # and Confirmation #: Helps link the incident to their booking.
- Reason: Be clear — theft, damage, threats, etc.
- Date Added: When the guest was banned.
- Notes: Staff observations, police report #, other important details.
